Finally published!Over the years the treatment program I've been working on, Neurofunctional Pain Management, has evolved into a refined protocol process that is exceptionally effective at relieving chronic pain from a range of pain conditions. I see it as a truly effective first line therapy for pain management before other options like invasive injections, implants, and surgeries. In the years I've been working in this field my favorite thing to do is ask the patient which method of treatment they'd prefer to start with. So far, it's been a landslide opinion that they'd want to start with the most effective option that is the least invasive or risky. Frozen ShoulderFrozen shoulder, also called adhesive capsulitis, causes pain and stiffness in the shoulder. Over time, the shoulder becomes very hard to move.Frozen Shoulder is more prevalentIn women, as approximately 70% of individuals who present with a frozen shoulder, are females.Among individuals 35-65 years old, with an occurrence rate of approximately 2-5% in the general population, In China and Japan, it's called the 50 year old shoulder due to its prevalence at that age.Within the diabetic population, with an occurrence rate of 20% .If an individual has had Frozen Shoulder (5-34% chance of having it in the contralateral shoulder at some point as well). Simultaneous bilateral involvement has been found to occur in approximately 14% of cases.Causes:Diabetes mellitus (with a prevalence of up to 20%)StrokeThyroid disorderShoulder injury (FOOSH, direct impact, dislocation)Dupuytren diseaseParkinson'sComplex regional pain syndromeAvascular necrosis (rare, but can occur)TuberculosisShortness of breath, severe cough, any compromises to the quality of the breathMetastatic diseaseRheumatismsMultiple joint involvementFever, chills, severe (inexplicable) painHistory of cancer (to the individual, or family)Any suspicion of a systemic pathology or condition. Treatment:Physiotherapy intervention is based on the stage and symptom presentation of frozen shoulder. Early intervention involves patient education, pain management and gentle stretching exercises. These exercises consist of specific shoulder mobilization exercises to maintain shoulder range of motion within a pain free window.In later stages physiotherapy intervention can consist of further stretching exercises, postural exercises, strengthening and pain management techniques. Pain management techniques can include acupuncture, heat, ultrasound, shockwave therapy and manual therapy.A home exercise program is also prescribed to maintain shoulder mobility and strength throughout the various phases. Right now, I'm in bed with very severe pain in my lumbar spine, where it feels like a 2X4 has been repeatedly hitting my lumbar spine VERY, VERY HARD.I can barely walk with my walker, and it's extremely painful to do so.I know this is the beginning of a type of an MS attack on my lumbar spine, and it can last from 3 days to just over a week.....and my oral surgery is in 5 days, with my pain management appointment the day after (pee in a cup, etc). I'm hoping for a very short attack, because I don't know how I would go through the pain clinic, give my urine sample, etc when I can't hardly walk.... and when I can walk, it's EXTREMELY PAINFUL to do so ๐ญ.I feel like this has to do with pain management because this type of pain is a chronic issue for me, but since they have repeatedly scolded and reprimanded me for telling them about these episodes, I have learned:1.) Never say anything other than "I'm fine" to them. Be "that perfect little patient" to them no matter what.But this rule will be virtually impossible if I have to have a wheelchair and assistance, if I show up unable to walk.Every time I have these types of MS attacks, the extreme lumbar pain with the 2X4 pain, is just the beginning of the attack. After a day or more, the lumbar pain will lessen, but I won't be able to use my legs for anything until the attack is over.My oral surgeon knows that I have MS and so does my PM doctor, but they've never seen me during an attack before because I do EVERYTHING IN MY POWER to cover it up and "fake being healthy".Right now, I'm not confident that I will be able to mask it and pretend "I'm fine" this time around. Healthcare Workers: There are some of your patients who are NOT "faking sick" or "feigning illness/injury".....some of us have become experts at "faking being well", simply saying "I'm fine" when we're really NOT fine, and we're experts at masking being healthy & well.Some of us "look bad on paper", but please get to know us and look past all of our numbers, past negative notes about us, etc....and PLEASE actually get to KNOW each complex patient! I know it's hard because of the tough job y'all have, but it leads to much better care for the patient, as well as a MUCH EASIER job when you & your co-workers are caring for us, the "complex patients", the "chronic pain patients", and others.#medicine #painmanagement #complexpatients #chronicpain #medical #pain #multiplesclerosis
